They come in three different sizes according to the size of your brace. They come standard with a 25° angle which can be adjusted - simply heat the insert up with a good hair dryer or a professional heat gun to make the thermommoldable plastic pliable. Then slip it back into the rear of the brace and apply the brace around your waist to auto-mold to the contours of your back.
